Spotty Teeth

Posted on 6/1/2026 by NC Pediatric Dentistry
Cute Black children smilingEver notice little white or brown spots on your child's teeth? It's pretty common, and there are a few reasons why it might happen. Let's explore what these spots are and what they might mean for your child's oral health.

What Are These Spots?

These spots, sometimes called enamel defects, are areas where the tooth enamel (the hard, outer layer of the tooth) hasn't formed properly. Think of it like this: the enamel is like the paint on a car. If the paint job isn't perfect, you might see some imperfections or discoloration.

Possible Causes

Several things can cause these spots to appear. Here are a few of the most common:

•  Enamel Hypoplasia: This happens when the enamel doesn't form properly during tooth development. It can be caused by things like illness, high fevers, certain medications (like some antibiotics), or even nutritional deficiencies during pregnancy or early childhood. It can affect a single tooth or multiple teeth.
•  Enamel Hypocalcification: This is when the enamel forms, but it isn't as hard or mineralized as it should be. This can make the teeth more susceptible to cavities and can result in chalky white or yellow spots. Fluoride exposure can also play a role.
•  Fluorosis: While fluoride is great for strengthening teeth, too much fluoride during tooth development can lead to fluorosis. This usually appears as very faint, lacy white lines or spots on the teeth. In more severe cases, the spots can be more noticeable and even brown. Often, fluorosis is due to swallowing fluoridated toothpaste or consuming well water with naturally high fluoride levels.
•  Early Cavities: Sometimes, what looks like a white spot is actually the beginning of a cavity. These "white spot lesions" are areas where the enamel is starting to break down due to acid produced by bacteria in plaque. They can appear near the gumline.
•  Trauma: An injury to a baby tooth can sometimes affect the developing permanent tooth underneath it. This can lead to spots or other enamel defects on the permanent tooth when it eventually erupts.

What To Do About Spotty Teeth

If you notice spots on your child's teeth, the best thing to do is schedule a check-up. A dental professional can determine the cause of the spots and recommend the best course of action.

What to Expect at the Dentist

During the appointment, the dental professional will:

•  Examine the teeth: They'll carefully look at the size, shape, and location of the spots.
•  Ask about medical history: They'll want to know about any illnesses, medications, or other factors that might have contributed to the problem.
•  Possibly take X-rays: This can help assess the overall health of the teeth and surrounding structures.

Possible Solutions

The "fix" depends on what's causing the spots. Here are some options:

•  Monitoring: If the spots are mild and don't pose a risk to the tooth's health, the dental professional might simply monitor them over time.
•  Fluoride treatments: Fluoride can help strengthen the enamel and prevent further decay.
•  Resin Infiltration: This involves applying a special resin to the affected area to fill in the porous enamel and improve its appearance.
•  Restorations: In some cases, a filling or other restoration might be needed to protect the tooth and improve its appearance.
•  Tooth Whitening: For some types of discoloration, whitening can improve the look of the teeth.

Spotty teeth are a common concern, but with the right diagnosis and care, we can help keep your child's smile healthy and bright!
